Incident Reporting

1. Summarize the incident, making notes on the name of the patient, the location of the incident, who was present, the procedure performed, dx that lead to that procedure and the outcome of that procedure.
2. Fax copy to Susan Starck at the Keane Insurance Group (numbers above).

Claims Reporting

1. Summarize the incident, making notes on the name of the patient, the location of the incident, who was present, the procedure performed, dx that lead to that procedure and the outcome of that procedure.
2. Call Susan Starck at 314.966.7733 or 800. 966.7731 and have the following information ready: Date received summons, who received it and when completed
3. Fax a copy of a summarization and Summons and Complaint to Susan Starck at 314.966.7797
